Nothing on this website is medical advice, and using it does not create a physician-patient relationship. The content here describes our practice and the services we offer. It is not a diagnosis, not a treatment recommendation, and not a substitute for a consultation with a qualified clinician who knows your history.
A physician-patient relationship is established only after you become a patient of the practice through our formal intake process.
If you are experiencing a medical emergency, call 911 or go to the nearest emergency department. Do not use this website, our forms, or text messaging to report an emergency or an urgent clinical concern. Those channels are not monitored continuously and are not appropriate for urgent matters.
Submitting an inquiry
Our forms ask about what you are looking for and how to reach you. They do not ask for health information, and you should not submit symptoms, diagnoses, medications, or test results through them. This website is not a secure channel for clinical information.
Submitting an inquiry does not obligate you to anything and does not obligate us to accept you as a patient. Availability is limited by the size of our practice, and we accept a small number of new members each month.
Text messaging
Paragon Total Wellness offers separate, optional text-message subscriptions. With your marketing consent, we may send recurring automated promotional messages about our memberships and services, event invitations, educational content, and follow-ups about your interest in Paragon. With your non-marketing consent, we may send replies to your inquiry, appointment confirmations and reminders, and scheduling updates. We send only the categories of messages you agreed to receive. Consent is not a condition of purchase, form submission, booking, membership, or receiving care.
- You must be 18 or older to opt in to text messages.
- Message frequency varies based on the subscriptions you choose, our campaigns, your inquiries, and your appointments.
- Message and data rates may apply, according to your mobile plan.
- Reply STOP at any time to cancel text messages. You may receive one confirmation of your opt-out; no further program messages will be sent unless you opt in again. To rejoin, complete the opt-in process again. Reply HELP for assistance, call (623) 244-4014, or email [email protected].
- We do not send diagnoses, test results, or other clinical information through this text messaging program. Do not send sensitive clinical information or urgent concerns by text.
- Carriers are not liable for delayed or undelivered messages.
How we handle mobile information, including that it is never shared with third parties or affiliates for marketing purposes, is described in our Privacy Policy.
No promises about results
We describe the care we provide and the testing we perform. We do not promise any particular health outcome, result, or improvement, and nothing on this site should be read as such a promise. Health outcomes depend on many factors specific to each person, and no clinician can guarantee them.
Services, pricing, and membership
Descriptions of memberships, assessments, and other services on this site are for general information. The specific terms of any service, including what is included, what is not, and what it costs, are set out in the separate agreement you receive before enrolling. Where this site and that agreement differ, the agreement governs.
Medications, outside laboratory work, imaging, specialist referrals, and hospital care are billed separately by those providers and are not included in membership unless the written agreement says so. We do not bill insurance.
Pricing and program details may change. We will honor the terms in effect at the time you enroll.

The text, images, logos, design, and other material on this site belong to Paragon Total Wellness or are used with permission. You may view and share this material for personal, non-commercial purposes. You may not copy, reproduce, or republish it commercially without our written permission.

Availability and accuracy

Nothing in these terms limits liability that cannot lawfully be limited, including liability for professional care actually provided to a patient.
Governing law
These terms are governed by the laws of the State of Arizona, without regard to its conflict of law rules. Any dispute arising from them will be brought in the state or federal courts located in Maricopa County, Arizona.
Changes to these terms
We may update these terms. When we do, we will change the effective date at the top of this page. Continuing to use the site after a change means the updated terms apply. An update does not expand a prior text-message consent or enroll you in promotional messaging.
