Pricing, stated plainly

The numbers are here before you book.

Paragon does not bill insurance for its own services. The team helps you use insurance strategically for outside labs, imaging and referrals.

Compass Assessment$3,250

A one-time, in-depth assessment with the complete workup and physician-written plan.

Membership$10K to $20K

Annual pricing depends on the tier. Both tiers have a monthly option. Paying for the year up front saves about 10%.

Full assessment creditWithin 3 months

Join membership within three months of finishing the Compass Assessment and the entire $3,250 fee is credited in full.

Value, made visible

What the fee actually buys.

The value is not a longer list of tests. It is one physician turning the pieces into one plan, then keeping that plan current as your life changes.

The Compass Assessment

One assessment. Nothing left for you to assemble.

Individual providers can sell you the measurements. Compass adds the physician work that turns those measurements into one useful clinical picture.

Explore the assessment and comparison
Illustrative separately purchased stack About $2,230 to $2,330 Current published prices across several providers
  • VO2 max, resting metabolism, body composition, and strength and power testingAbout $450 to $500
  • 80+ biomarker panel with review, plus separately purchased Scottsdale mobile collectionAbout $685
  • 90-minute new-patient physician consultation plus a 45- to 60-minute follow-upAbout $750 to $850
  • One published custom fitness and nutrition plan$300
$3,250, one time. One schedule, one physician, and one plan built from the complete picture.

Already have recent labs from a testing service? Bring them. Compass can build them into the picture.

For context: A Scottsdale hospital executive-health program requires a full-day base package whose price is not shown online, then lists optional specialty packages from $1,200 to $3,650 in addition. Compass is $3,250, completed close to home, and the full fee credits toward membership within three months.
Beyond individual services

Then there is the column with no prices.

Some of the most important work is not a scan, a lab, or an appointment. It is having one physician maintain context when the situation changes.

Explore physician coordination and continuity
A personal physician who knows your whole story
Vetting specialist referrals and recommendations
Coordinating second opinions
Reviewing external treatment plans
Advocacy and support during hospitalization
Continuity of care after discharge
A plan that adapts whenever your life changes

These services are not typically priced or sold one at a time. At Paragon, they run through the Blueprint, one plan a physician writes from the whole picture.

Ongoing membership

One system that stays current.

Both memberships combine physician continuity with a recurring assessment cadence. Public alternatives package the pieces differently, so there is no clean like-for-like assembled price.

Ascent$10,000/year

Physician-led concierge care with two comprehensive evaluations each year.

Explore what Ascent includes
  • Premium medical and primary care, year round
  • Two Compass evaluations each year
  • Hormone and weight-loss oversight
  • Eligible-member genetic screening, plus adaptive planning and navigation
Public market contextNo like-for-like price

A single assembled total would rely on overlapping programs and assumptions, so we do not present one as a market fact.

Member perspective

What care feels like when the plan is personal.

Three Google reviews, shared in their own words. The common thread is not more data. It is finally having a physician who turns that data into direction.

★★★★★Google review

I started at Paragon Total Wellness over a year ago and it had been transformative. I have been working with Dr Cushman and Rachel to review and my overall health. I have improved my diet, sleep, exercise, rest and feel a higher sense of wellbeing.

I appreciate the ability to work with them to focus on my health without being sold “supplements” or “IVs” that have no clinical value. For those people that want a hands on approach to comprehensive proactive healthcare -Paragon is awesome!

★★★★★Google review

Taylor and Rachel are incredible health care professionals. They are extremely knowledgeable and passionate about the care they provide. Taylor provided both my wife and I a very detailed health plan that was specific to our profile.

They are very much on the forefront of medical care and lean towards preventive care and a healthy lifestyle. They’re both extremely responsive and have been great on executing our individual health plans. Wish I would have started this 10 years ago.

Common questions

The details, stated plainly.

Do you take insurance?
No. Paragon is a membership practice and we do not bill insurance for our own services. We do help you use your insurance intelligently for labs, imaging, and outside referrals, where it often covers a meaningful share.
What is included in membership, and what is not?
Included: your primary care with a dedicated physician, the full testing workup, a written plan with quarterly recalibration, direct access, and coordination with specialists. Not included, and worth knowing up front: medications, imaging, outside specialists, and hospital care are billed separately by those providers.
What if I am not ready for a membership?
Start with the Compass Assessment: the complete workup and a physician-written plan, as a one-time engagement. If you join membership within three months, the full Compass fee is credited toward it. If you do not, the plan is yours to keep and take anywhere.
I already have labs, a scan, or wearable data. Is that wasted?
The opposite. Bring your existing results, whatever you have invested in, and they become the starting point rather than being repeated. The work we add is reading everything together and turning it into one plan.
What happens after I answer the questions?
If your timing is soon, the booking calendar appears immediately so you can choose a short fit conversation. If you are planning for later or researching, the team follows the timing you selected. No sales script and no pressure.
Where are you located, and do I need to live nearby?
The practice is at 7500 N Dobson Rd Suite 101 in Scottsdale, and visits happen in person, with some labs available at home. Members split their year between Phoenix and elsewhere more often than you would think; tell us your situation in the questions above.
Is this medical advice?
No. Nothing on this page is medical advice, and using this site does not create a physician-patient relationship. If you are experiencing an emergency, call 911. This page and our forms are not for urgent clinical matters.

This questionnaire helps the Paragon team prepare the right follow-up. It is not medical advice, it is not a diagnosis, and completing it does not create a doctor-patient relationship. Paragon operates on a membership model and does not bill insurance.
