Privacy Policy

What this policy covers

This policy applies to information you give us through this website, including our consultation request form and our text message program. Paragon Total Wellness, a physician-led practice located at 7500 N Dobson Rd Suite 101, Scottsdale, Arizona, is responsible for that information.

This policy does not govern protected health information created or received in the course of providing care. That information is handled under the practice's Notice of Privacy Practices and applicable health privacy law, which are separate from this policy.

What we collect, and what we do not

What we collect when you contact us:

• Your name, email address, and phone number.
• Answers to short qualification questions covering what you are looking for, where you are located, whether a membership model works for you, and your timing.
• Any message you choose to write to us.
• Whether you consented to receive text messages, the exact consent wording shown to you at the time, and the date and time of that consent.
• Basic technical information about your visit, including the advertisement or link that brought you here.

How we use your information

• To respond to your inquiry and answer your questions.
• To schedule, confirm, remind you of, and reschedule appointments.
• To understand which advertisements and pages bring people to us, so we can improve them.
• To meet legal, regulatory, and recordkeeping obligations.

We do not use the information you submit here to make any clinical determination about you.

Text messaging

If you provide your mobile number and check the consent box, we may send the types of messages described in that consent, including replies to your inquiry, information about our services, promotional follow-ups, and appointment confirmations, reminders and scheduling updates. Promotional messages require consent that includes promotional messaging; updating this policy does not expand an earlier consent. Consent to receive text messages is optional. It is never required to submit a form, to book an appointment, to become a patient, or to receive any service from us.

Message frequency varies with your inquiry, appointments and replies. Message and data rates may apply. You may opt out at any time by replying STOP to any message. Reply HELP for assistance, or call us at (623) 244-4014.

We do not send test results, diagnoses, or other clinical information through this text messaging program.

Who we share information with

We do not sell, rent, trade, or otherwise distribute your personal information. We share it only in these situations:

• Service providers who work on our behalf. This includes our website host, our customer relationship management and messaging provider, and the marketing agency that operates our advertising. They may access your information only as needed to perform their work for us and may not use it for their own purposes.
• Advertising measurement. We use Google advertising measurement to understand which advertisements lead to visits and inquiries. Where enhanced measurement is enabled, an email address may be converted to a one-way hashed value for matching by Google. Hashing is not encryption or anonymity: Google may match the value with information it already holds. We do not authorize sharing clinical information for advertising measurement.
• Optional Meta measurement. Where enabled and with your permission, Meta Platforms, Inc. may receive limited technical information from public marketing pages through the Meta Pixel, such as a page address, browser information, IP address and cookie or advertising identifiers. A successful inquiry submission may be reported as a Lead event to measure Facebook and Instagram advertising. Meta may associate that information with your account and use it under its own privacy policy. We do not send your name, email address, telephone number, questionnaire answers, qualification score, message contents, clinical information, appointment details or Blueprint activity to Meta. We do not track calendar interactions, appointments, document activity or payments with Meta. If navigation interrupts delivery of a consented inquiry event, the booking page may finish sending that same event without sending booking information.
• When the law requires it, such as a valid subpoena, court order, or legal obligation.

We do not upload your information to advertising platforms to build audiences, and we do not use it to create lookalike or similar-audience targeting.

Cookies and advertising measurement

This site uses Google advertising measurement and parameters attached to advertising links to understand website traffic. Advertising measurement is not necessary to use the questionnaire or contact us. We do not use health-related audience targeting.

Optional Meta advertising measurement on our public funnel pages loads only after you choose Allow in the advertising measurement notice. You can decline or withdraw permission through Advertising privacy choices at the bottom of those pages. We honor Global Privacy Control by not enabling optional Meta measurement. Declining optional measurement will not prevent access to the questionnaire, Blueprint or booking options.

Consent to receive text messages is separate from advertising-measurement permission. Allowing cookies does not authorize us to share sensitive information or bypass an advertising platform’s restrictions.

You can control cookies through your browser settings. Blocking them will not prevent you from using this site or contacting us.

How we protect your information

Information submitted through this site is transmitted over an encrypted connection and stored in access-controlled systems. Access is limited to the people at Paragon Total Wellness and its service providers who need it to respond to you. No method of transmission or storage is perfectly secure, so we cannot guarantee absolute security, but we take reasonable measures appropriate to the sensitivity of the information involved.

How long we keep it

We keep inquiry records for as long as needed to respond to you and to maintain reasonable business records, and then delete or de-identify them. Records of text message consent and opt-out are kept for as long as required to demonstrate compliance with messaging regulations, which may be longer than the inquiry itself. If you become a patient, records created in the course of your care are retained under the practice's medical records policy rather than this one.

Your choices

• Text messages: reply STOP to any message to opt out at any time.
• Email: use the unsubscribe link in any email, or ask us directly.
• Access, correction, or deletion: email [email protected] or call (623) 244-4014 and we will honor reasonable requests, subject to any records we are legally required to keep.

Depending on where you live, you may have additional rights over your personal information. Contact us using the details below and we will respond.

Children

This website is intended for adults. We do not knowingly collect information from anyone under 18 through this site. If you believe a minor has submitted information to us, contact us and we will delete it.

Changes to this policy

If we change this policy we will update the effective date at the top of this page, and for significant changes we will make the update noticeable on the site. Changes to this policy do not by themselves grant consent to optional advertising measurement.
